$250.00 with a nonrefundable deposit of $100.00 to be paid prior meeting. Three hour meeting maximum with one free follow up call or visit. We will meet at a convenient public facility.
A check off list is provided so we can thoroughly assess your health care options, once the deposit has been paid, you may download list.

If you need to cancel – please send an email a week in advance. The deposit is valid, as long as you reschedule at the time of cancellation. You may reschedule only once.

My business email is provided under the Consultation Meeting option for your convenience.  Please provide contact information as requested: include your name – phone number and best time to call.  I will return calls within 48 hours of receiving your notice.

A consultation form will be sent to you via e-mail upon completion of the payment process. Please make sure to bring the printed and filled-out form on the day of your consultation.
